Job Summary
The University of Alberta Hospital (UAH) offers specialized diagnostic and treatment services to adult patients, as well as specialty ambulatory clinics across the campus. This Social Worker II position is part of the Social Work team and is responsible for delivering social work services to adult patients across inpatient units, collaborating with team members, patients and families in a rapid‑paced environment, ensuring trauma‑informed, equitable care.
Responsibilities- Provide a range of social work services to patients and their families as part of an inter‑disciplinary team.
- Conduct psychosocial assessments, resource‑based interventions, individual, group, and family treatment, risk screening, counseling, case management, and coordination.
- Assist with transition or discharge planning, prevention, promotion and public awareness activities as appropriate.
- Collaborate with colleagues, patients and families to ensure trauma‑informed, patient‑centered care.
- Liaise with stakeholders and consult with other care providers to coordinate care.
- Completion of a bachelor’s degree in Social Work.
- Active or eligible for registration with the Alberta College of Social Workers (ACSW).
- Minimum of two years’ experience as an RSW in acute healthcare settings.
- Current Basic Life Support (BLS‑HCP) certification.
- Experience working with diverse cultural groups and vulnerable populations, demonstrating trauma‑informed, patient‑centered care.
- Strong professional judgment, time‑management skills, critical thinking and problem‑solving abilities.
- Ability to ensure concise, timely and accurate communication, documentation and collaboration with colleagues and patients.
- Self‑direction for independent work and positive contribution to an interdisciplinary team.
- Master of Social Work (MSW) degree.
- Knowledge and ability to use computer applications such as Connect Care.
- Experience with relevant legislation (including Personal Directives Act and the Adult Guardianship and Trusteeship Act), conflict resolution, and complex placement processes.
