Join the Northern Alberta Program as a Pharmacist I, collaborating with a multidisciplinary team to support patients with HIV. This role focuses on optimizing medication therapy and providing comprehensive patient care in an ambulatory setting.
At the Kaye Edmonton Clinic, you will work as a key member of a team that includes physicians, nurse practitioners, and social workers to enhance the health of individuals living with HIV. Responsibilities include initiating therapy, addressing drug-related issues, and ensuring continuity of care through community pharmacies. Your efforts will support newly diagnosed patients and those requiring re-engagement with their healthcare.
Key Responsibilities:
• Initiate and adapt medication therapy for patients
• Optimize therapy and resolve drug-related issues
• Coordinate continuity of care with community pharmacies
• Participate in rapid HIV antiretroviral treatment initiation
• Provide medication counselling and drug information
Requirements:
• Bachelor's degree in Pharmacy required
• Registration with Alberta College of Pharmacy
• Minimum 1 year of clinical pharmacist experience
• Additional prescribing authorization necessary
• Previous HIV or infectious disease experience preferred
