Join Winnifred Stewart as a Support Coordinator, providing direct support to individuals with disabilities while ensuring their needs are met in a community setting. This position emphasizes person-centered planning and high needs care in your daily operations.
In this role, you will develop personalized plans with individuals and their support circles, ensuring they achieve their life goals. You will assist in more complex situations, encourage choice-making, and support health and well-being for individuals in care. You will also strictly adhere to medical protocols while creating recreational opportunities within the community.
Key Responsibilities:
• Develop individualized support plans with clients
• Encourage rights and choice-making for individuals
• Provide direct care for high-needs or medically fragile individuals
• Monitor well-being and respond to condition changes
• Create clear documentation and reports as required
Requirements:
• Diploma or equivalent post-secondary education
• 2+ years’ experience with diabetes, epilepsy, dementia
• Effective communication and mentoring skills
• Training in Mindful Hearts, MANDT, or PBS preferred
• Ability to balance competing priorities and demands
