Program Overview

The Clinical Integration Pharmacist (CIP) of the Edmonton North Primary Care Network (PCN) is an integrated primary care role embedded within the physician clinic. Serving patients experiencing health and social inequities in Edmonton, the pharmacist works with interdisciplinary teams to support complex physical, behavioural health, medication-related, and social needs. The pharmacist coordinates clinic medication management systems; serves as a dedicated clinical resource in addictions medicine, infectious diseases, and chronic disease management;

supports transition of care, including hospital discharge follow-up; and liaises with community pharmacies and other healthcare partners to improve medication access, continuity, adherence, and therapeutic outcomes.

Position

The Clinical Integration Pharmacist provides direct patient care, medication management expertise, and leadership in the development of safe and effective medication systems within the clinic. The role supports patients through medication assessment, prescribing and deprescribing activities, chronic disease management, addiction medicine, transitions of care, medication access, patient education, and interdisciplinary consultation.

The Pharmacist also contributes to quality improvement initiatives, program development, learner preceptorship, and the ongoing evolution of integrated primary care pharmacy services.

Key Responsibilities

  • Provide comprehensive medication assessments, prescribing, deprescribing, monitoring, education, follow-up, and medication administration activities within scope of practice.
  • Support patients with complex medication needs, including chronic disease management, mental health concerns, substance use disorders, polypharmacy, pain management, and care transitions.
  • Coordinate medication-related care across healthcare settings and assist patients in accessing medication coverage programs, compassionate access pathways, and community supports.
  • Collaborate with physicians, nurses, community pharmacies, specialists, and external partners to optimize medication therapy, address medication-related barriers, and support continuity of care.
  • Provide medication-related consultation, education, and clinical support to interdisciplinary team members and contribute to patient and provider learning opportunities.
  • Support clinic medication management systems, medication safety practices, policies, workflows, and regulatory compliance requirements.
  • Participate in quality improvement initiatives, program planning, service development, evaluation activities, health promotion, harm reduction, and learner mentorship to support the ongoing growth of integrated pharmacy services

    Education

  • Completion of Bachelor of Science in Pharmacy (BSc. Pharm.) or Doctor of Pharmacy (PharmD)
  • Active registration with the Alberta College of Pharmacists (must be able to provide proof of active registration and practice)
  • Additional Prescribing Authority (APA) and Authorization to Inject
  • Experience

  • Minimum three to five years of pharmacy experience, preferably in a clinical, primary care, or interdisciplinary healthcare setting.
  • Demonstrated expertise in providing comprehensive pharmaceutical care, including medication assessment, prescribing, deprescribing, monitoring, and patient education.
  • Experience and advanced clinical competencies in addictions medicine and the care of vulnerable or marginalized populations are strongly preferred.
  • Experience supporting patients with complex health needs, including chronic disease management, mental health concerns, substance use disorders, polypharmacy, chronic pain, geriatrics, and care transitions.
  • Strong knowledge of pharmacotherapy, medication safety, and evidence-informed clinical practice.
  • Experience collaborating within interdisciplinary healthcare teams and coordinating care across healthcare settings and community partners.
  • Knowledge of medication coverage programs, community resources, and supports that improve access to care for patients experiencing financial or social barriers.
  • Experience in program development, quality improvement, service evaluation, or implementation of evidence-based initiatives is an asset.
  • Experience precepting, mentoring, or supporting pharmacy learners is an asset.
  • Experience in primary care and/or integrated team-based care is an asset.
  • Additional specialty certifications or training relevant to primary care practice (e.g., geriatrics, diabetes, tobacco cessation, respiratory care, addictions, or other pharmacotherapy specialties) are considered assets.
  • Current certification in Standard First Aid and CPR (Basic Life Support – BLS)
