As Clinical Director, you'll lead a dedicated group of Nurse Practitioners and Physician Assistants while ensuring top-notch care for seniors in their homes. Your tasks will include conducting assessments, developing care plans, and enhancing the quality of life for older adults. Engage with community partners and optimize healthcare delivery processes to improve overall patient satisfaction.

Key Responsibilities:

• Establish patient relationships through Welcome Visits

• Ensure completion of preventive care and documentation

• Provide coverage during provider absences for continuity

• Respond urgently to medical needs to avoid hospitalizations

• Oversee team training and professional development

Requirements:

• Active CA APRN license; CA 103 NP preferred

• Minimum three years in geriatric clinical settings

• 3 years supervisory experience in healthcare

• Advanced diagnostic skills and medical knowledge

• Strong ability to work in diverse healthcare settings
