Community Patient Navigator Position KDFN

Elevate health services as a Community Patient Navigator for Kwanlin Dün First Nation at the Natsékhi Kų̀ Health Centre. This position focuses on advocacy and resource navigation for clients with complex care needs.

Candidates should possess a Bachelor of Social Work Degree and two years of experience in a relevant field. The role involves close collaboration with healthcare professionals and community members to ensure comprehensive care delivery and wellness planning. Strong communication and organizational skills are essential for success in this supportive position.

Key Responsibilities:

• Advocate for clients’ rights and access to services

• Facilitate health resource navigation for individuals

• Enhance client health literacy through education

• Develop comprehensive wellness plans in collaboration with teams

• Manage case documentation and reporting effectively

Requirements:

• Bachelor’s Degree in Social Work with 2 years of experience

• Skills in counseling and crisis intervention

• Knowledge of child development and trauma

• Strong organizational skills and confidentiality standards

• First Aid & CPR-C certification or willingness to obtain
