Social Worker for Community Mental Health

As a vital part of the interprofessional team, you will lead engagement and service planning for adults facing serious mental health conditions. Your expertise will empower individuals through advocacy, comprehensive assessments, and coordination of various rehabilitation services.

Key Responsibilities:

• Develop therapeutic relationships with clients and families

• Coordinate and monitor treatment and recovery plans

• Implement and evaluate individual goals for recovery

• Advocate for clients’ rights and preferences

• Provide direct clinical services and health education

Requirements:

• Master's Degree in Social Work

• Registered with Ontario's Social Work College

• One year experience in mental health services

• Experience in diverse community settings

• Valid Ontario G Class driver’s license
