Care Rx is looking for a Clinical Consultant Pharmacist to join our Edmonton team. This position has a flexible schedule which gives the ability for work life balance. The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities.
The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal Care Rx policies, and the Ministerial Order for Clinical Pharmacy Services.

Role Accountabilities:
Act as aliaisonbetweenlong term care homesand retirement communities and Care Rx Pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support
Travel to LTC homes and retirement communities
Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/communityleadership teams and pharmacy teams to develop/maintain strong partnerships
Complete medication reviews in currently serviced retirementcommunities and long-term care homesserviced by Care Rx Pharmacy
Complete all required documentation and billing processes as required by MOHLTC, Alberta College of Pharmacy (ACP), and other federal and provincial standards and regulations
Provide education to Residents and staff on various health-related topics including theMedsCheckprogram and attend community-based wellness fairsand other eventsas required
Conducteducation sessions within
Long Term Care Homes and Retirement Communities within scope of practice, as applicable…
