We are seeking a Social Worker II to join our Central Zone Transition Social Work Team in the Wetaskiwin area.
This role provides professional social work services in clients’ homes, continuing care facilities, and hospital settings. As part of a multidisciplinary team, you will:
Conduct psychosocial assessments, identify unmet needs and develop care plans, provide social navigation and resource-based interventions, collaborate with clients, families, and decision-makers to achieve client goals.

- Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Required Qualifications:
- Strong communication and collaboration skills, ability to work independently in diverse settings (homes, congregate living, facilities, hospital) professional judgment, initiative, and time management, proficiency in Microsoft Office Knowledge of seniors’ benefit programs and relevant legislation:
Guardianship and Trusteeship Act, Personal Directive Act, Enduring Power of Attorney Act.
- Experience in Continuing Care settings.
- Familiarity with Connect Care.
- Palliative Care experience.
- Knowledge of community resources, referral pathways, and interagency collaboration.
