Alberta Health Services invites applications for a Social Worker II in the Wetaskiwin Hemodialysis Unit. You will join an inter‑disciplinary team providing psychosocial assessments, counseling, and resource navigation for patients with kidney disease and their families.
Responsibilities include risk screening, case management, discharge planning, and health promotion in a fast‑paced renal care environment. BLS‑HCP and ACSW eligibility are required.
