As a Health Care Aide at Age Care, you will join a collaborative care team, working under the guidance of a Licensed Practical Nurse. This position will help create comprehensive care plans while actively supporting residents’ daily activities and health monitoring. Bring your experience in community-based care to create a welcoming environment for residents.

Key Responsibilities:

• Deliver focused care tailored to each resident’s needs

• Observe and communicate changes in resident conditions

• Collaborate on care plan development and implementation

• Support residents in community participation and mobility

• Report safety hazards to ensure resident well-being

Requirements:

• Graduate of recognized Health Care Aide program

• Experience with seniors in health service settings

• Strong organizational and time management skills

• Basic Cardiac Life Support and First Aid certification preferred

• Complete a current Vulnerable Sector Check
