Registered Nurse - Palliative/End of Life, Continuing Care
Assisted Living Alberta is dedicated to enhancing care in the community, helping people be healthy, well and independent in their homes and communities. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life.
As a self‑directed team member in Continuing Care, the Registered Nurse works collaboratively with the health care team to assess needs, deliver appropriate therapeutic nursing services within professional practice standards, coordinate resources, establish goals for client delivery, and evaluate outcomes. The role also involves case management, direct client care, and serving as a clinical consultant to the team. The RN participates in staff development and mentors student clinical experiences.
This position is based out of the St. Albert Home Care office with travel to other home care network offices as required and focuses on supporting Palliative/End of Life clients in a case‑management role.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, ensuring their safety and well‑being. You will utilize nursing processes, critical thinking, problem solving and decision making, teach, counsel and advocate on behalf of patients and their families, and play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.
You will provide safe, quality patient and family‑centered care while reflecting the shared vision and values of the organization.
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support – Health Care Provider (BCLS‑HCP) certification.
- Completion of free courses if you have not practiced as a Registered Nurse within the last five (5) years:

Required Qualifications
- Two years of recent nursing experience (within the last 5 years) or completion of a refresher program within 2 years.
- Self‑directed and the ability to work independently and use sound professional judgement.
- Excellent organizational skills to prioritize and efficiently organize many details in a timely manner.
- Communication and assessment skills to work cooperatively and effectively with the healthcare team, clients and their families in a fast‑paced environment.
- Proficient computer knowledge, including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet, with sufficient typing speed to meet workload demand.
- Physically capable of performing nursing tasks, including client transfer and lifting/carrying medical bags weighing approximately 25‑30 lbs, in various home environments and weather conditions.
- Baccalaureate degree in nursing preferred; skills and experience relevant to nursing practice in the community sector considered.
- Palliative care experience.
- Previous Continuing Care and/or geriatric experience.
- Experience with Connect Care and the RAI Home Care assessment tool.
- Two years of current Case Management Experience in Continuing Care.
