Key Responsibilities:
• Assess client needs and craft care plans accordingly
• Deliver and coordinate nursing services per program guidelines

• Evaluate and adjust care plans based on patient outcomes
• Collaborate within a multidisciplinary home-care setting
Requirements:
• Graduate from an accredited nursing program
• Eligible for registration with the College of Registered Nurses of Alberta
• Current BCLS-HCP certification
• Willingness to meet on-call response time
• Ability to perform physical demands of the role
