Key Responsibilities:

• Conduct ongoing patient assessments and support services

• Act as a liaison between patients and providers

• Maintain clear records in the electronic patient system

• Offer continual support on disease-related inquiries

• Coordinate logistics with healthcare facilities

Requirements:

• Diploma or Bachelor’s degree in a related field

• Active Nurse registration (RN or RPN/LPN)

• 2-5 years in healthcare or related fields

• Case management experience in medical settings preferred

• Knowledge of French and/or English applicable
