In this self-directed role, you will engage with patients and their families, delivering personalized nursing services and coordinating their care plans. Through collaboration with healthcare providers and mentoring students, you contribute to a supportive environment that enhances the quality of care. Your expertise in case management will greatly impact both individual patients and the wider community.
Key Responsibilities:
• Assess client health needs across various settings
• Provide high-quality therapeutic nursing services
• Develop and implement individualized care plans
• Mentor students and contribute to staff education
• Collaborate with multidisciplinary teams for care coordination
Requirements:
• Completion of a registered nursing education program
• Eligibility for registration with the College of Registered Nurses of Alberta
• Current Basic Cardiac Life Support certification
• Preferred experience in palliative and community care
• Two years' case management experience in a continuing care setting
