Job Overview  The Lead Licensed Practical Nurse (LPN) is responsible for assessing, planning, implementing, and evaluating resident care. The LPN provides support and direction to Health Care Aides and works as an active member of an interdisciplinary team. The LPN is accountable for safe and competent nursing care within the scope of LPN practice at Grace Manor or Supported Housing, reporting to the respective manager.
Accountabilities  Monitoring and Assessment   Perform and contribute to the initial and ongoing nursing assessment of assigned residents.
Collect, document and report health information from objective and subjective sources to identify residents’ health needs, monitor changes in health status using critical judgment, and assess residents’ ability to function independently in their home environment.
Document timely and accurate reports of data collection, interpretation, planning, implementation and evaluation of care in accordance with Salvation Army policies, procedures and regulatory requirements.
Care Planning   Analyze information and identify priorities, initiate and/or collaborate in developing plans of care to meet resident needs.
Provide, initiate, and reinforce teaching using developed educational materials.
Participate in developing discharge plans and coordinate resources to ensure continuity of care.
Identify and initiate interventions within the LPN’s legislated scope of practice.
Care Plan Implementation   Implement nursing treatments and interventions using evidence‑based knowledge to accomplish plans of care.
Assist residents with physical requirements and activities of daily living.
Evaluation   Observe, report and document changes in resident status and deviations from anticipated response/outcome.
Evaluate and suggest modifications to care plans on an ongoing basis.
Make recommendations to the health care team, AHS case manager, social worker (if applicable) and the manager regarding changes to treatment plans.
Assess and monitor unstable residents throughout the shift; assume care or consult with the manager and arrange transfer to acute care if necessary.
Communicate promptly with AHS case manager and/or physician when there is a change in a resident’s status.
Documentation   Record complete and accurate relevant data for each resident in a retrievable form according to departmental policies.
Professional Responsibilities   Participate in programs to develop and maintain clinical competency and promote professional growth.
Participate as a team member in quality improvement activities and nursing practice committees.
Demonstrate a professional image to residents, peers and other health care providers and the public.
Serve as team leader when needed, providing direction and supervision to HCA care staff in a fair and equitable manner.
Provide nursing leadership to team members, residents and families, directing nursing care and crisis intervention as needed.
Report unskilled or unprofessional conduct and compliance violations to the manager promptly.
Participate in research and staff development projects.
Participate in orientation of new employees/students and precept students as assigned.
Cooperate and participate fully in the occupational health and safety system.
Identify and report hazards and incidents with potential for injury to self or others.
