This role is offered on a one-year contract with possible extension, reporting to the Medical Director. As a Nurse Practitioner, you'll manage care for 15-20 unattached clients, ensuring they receive timely symptom management and support. Engage in critical discussions regarding goals of care with patients and their families while working closely with healthcare professionals.
Key Responsibilities:
• Provide regular home and virtual visits for patients
• Monitor and manage symptoms, prescribing medications
• Conduct care discussions with patients and families
• Attend handover and rapid rounds, either virtually or in person
• Collaborate with healthcare colleagues for comprehensive care
Requirements:
• 3-5 years in Hospice/Palliative End-of-Life Care
• Current license with Alberta’s College of Registered Nurses
• Strong communication and interpersonal skills
• Community-based experience considered beneficial
• Must hold a valid Alberta Driver’s License
