Lead Licensed Practical Nurse at Salvation Army

As a Lead Licensed Practical Nurse, you will assess and evaluate resident care in Grace Manor or Supported Housing. You'll work closely with Health Care Aides, providing mentorship, implementing care plans, and ensuring continuity of care. Key Responsibilities:

• Conduct ongoing nursing assessments and document health information

• Develop and implement individualized care plans for residents

• Monitor changes in resident health status and adjust care as needed

• Train and support Health Care Aides in daily tasks

• Collaborate with an interdisciplinary team to enhance resident wellness

Requirements:

• Graduate from an accredited Licensed Practical Nurse program

• Current registration with CLPNA and BLS certification

• Minimum of two years' experience in a care setting preferred

• Excellent communication and critical thinking skills

• Compassionate approach and attention to detail
