Working through ENPCN, you’ll focus on a multidisciplinary approach to refugee health.
Your role will include assessing patients, establishing care plans, and providing education tailored to individuals with chronic conditions. With an emphasis on adaptability and responsiveness, you’ll ensure that care aligns with patient needs in a culturally sensitive manner.
Key Responsibilities:
• Assess and develop management strategies for patients
• Create and implement patient care plans
• Collaborate with medical professionals and community entities
• Provide guidance on health education and chronic disease management
• Support patients' transitions to community healthcare
Requirements:
• Active CRNA registration as a Nurse Practitioner
• Master’s degree in Nursing required
• Minimum three years of relevant clinical experience
• Advanced skills in chronic patient care management
• Valid BLS CPR certification and immunization records
Utilize your nursing expertise to support diverse populations at the New Canadians Health Centre.
