Provide vital home healthcare services as a Home Health RN s role focuses on compassionate nursing care for seniors in their homes, promoting independence and wellness.
As a Home Health RN, you will work directly with elderly participants, coordinating care plans alongside an interdisciplinary team.
Your role will include conducting nursing assessments, delivering interventions, and ensuring medication administration, all while maintaining a healthy environment. Flexibility is key, as you will enjoy a supportive work culture and a focus on care over administrative tasks.
Key Responsibilities:
• Conduct home health nursing assessments to determine participant needs
• Actively participate in interdisciplinary team meetings
• Administer medications and perform ordered treatments
• Instruct participants and caregivers on self-care and prevention
• Collaborate with physicians for smooth care transitions
Requirements:
• Graduate of an accredited nursing school
• Unencumbered RN license required
• One year of clinical nursing experience
• Reliable transportation for home visits
• CPR and First Aid certification within 180 days
