This role with the Short Term Intervention Team involves delivering direct care, assessing client conditions, and establishing individualized care plans. You'll work closely with clients of all ages and stages, ensuring that clinical standards are not just met but exceeded. Engage in community visits and manage case responsibilities for optimal client outcomes.
Key Responsibilities:
• Implement comprehensive nursing assessments for clients
• Develop and evaluate care plans addressing nursing needs
• Provide triage and clinical support as required
• Facilitate home visits utilizing specialized skills
• Advise team members on nursing care best practices
Requirements:
• Accreditation from a recognized nursing education program
• Registration and practice permit eligibility with CRNA
• Certification in Basic Cardiac Life Support
• Experience in palliative or continuing care settings
• Strong organizational and interpersonal skills
