Bigstone Health Commission is seeking a compassionate and motivated Licensed Practical Nurse (LPN) to join our multidisciplinary Diabetes Program team. Working under the guidance of the Diabetes Program Coordinator, the successful candidate will support diabetes prevention, education, chronic disease management, and community outreach initiatives. This role is ideal for someone committed to client‑centred care, cultural awareness, and collaborative practice within First Nations communities.
Duties & Responsibilities:
Provide nursing care and education to clients living with Type 1, Type 2, gestational diabetes, and pre‑diabetes within the LPN scope of practice.
Conduct diabetes screening, risk assessments, and clinical measurements, including:
Blood pressure, weight, BMI, and foot assessments, review of blood glucose records.
Blood glucose monitoring
Medication adherence
Healthy eating and physical activity
Recognition and management of hypo/hyperglycemia
Participate in care planning, follow‑up appointments, and telephone outreach.
Maintain accurate clinical documentation and client confidentiality.
Assist with community screening events, workshops, support groups, and wellness initiatives.
Perform other related duties as assigned.

Strong assessment, organizational, and clinical decision‑making skills.
Excellent communication, interpersonal, and teamwork abilities.
Ability to work independently and within a multidisciplinary team.
Education & Experience:
Graduate of an approved Licensed Practical Nursing program.
Current registration with the College of Licensed Practical Nurses of Alberta (CLPNA) .
Minimum one (1) year of LPN experience preferred.
Current BLS (Level
C), First Aid, WHMIS, and TDG certifications, or willingness to obtain.
Valid Alberta Class 5 driver’s licence.
Experience working with First Nations communities is an asset.
