As a Licensed Practical Nurse (LPN), you will report directly to the Director of Care and be an integral part of a dedicated interdisciplinary team. Your focus will be on delivering goal-directed care tailored to each resident's needs while ensuring compliance with provincial standards. Collaborate with dynamic professionals to create personalized care plans and make meaningful contributions to enhance residents' well-being.
Key Responsibilities:
• Conduct comprehensive resident assessments and maintain care documentation
• Implement and evaluate individualized care plans
• Assist with transitions for residents during move-ins and discharges
• Manage medications and respond to medical emergencies
• Coordinate care with service providers and address family concerns
Requirements:
• Current LPN registration in British Columbia
• Experience in long-term or senior care preferred
• Strong clinical knowledge and communication skills
• Current CPR and First Aid certification
• Proficiency in electronic documentation systems
