In your role as Transition Coordinator at Assisted Living Alberta, you will leverage your nursing skills to provide essential services to the community. You will assess patient needs, develop comprehensive care strategies, and coordinate with health professionals to ensure seamless transitions for patients. This highly collaborative role will involve critical decision-making and enriched liaison work to connect patients with necessary resources.
Key Responsibilities:
• Perform detailed assessments for service planning
• Coordinate care across various health facilities
• Provide ongoing communication with healthcare teams
• Document care processes for continuity
• Assist and educate patients and families in care options
Requirements:
• Active or eligible for CRNA registration
• Five years’ facility-based nursing experience
• Two years’ nursing experience within the community
• Strong conflict resolution and communication skills
• Ability to manage competing priorities in stressful environments
