Intensive Case Management Support Worker

Join The Mustard Seed as a Follow-Up Support Worker, providing essential support in Edmonton. Your focus will be on client advocacy, community connections, and housing stability.

In this Full-Time role, you will be a part of the Intensive Case Management Team, helping clients navigate their journey towards sustainable independence through a Housing First approach. You will work closely with clients, build relationships, and provide comprehensive support tailored to individual needs. Your efforts will contribute to improving their life skills and access to essential services.

Key Responsibilities:

• Manage an active caseload with ongoing home visits

• Build client recovery plans centered on their strengths

• Connect clients to vital community services and resources

• Provide crisis intervention when necessary

• Deliver life skills training focusing on essential areas

Requirements:

• Preferred degree in Social Work or a related discipline

• Experience with vulnerable populations and housing initiatives

• Familiarity with harm reduction principles

• Strong advocacy and communication skills

• Valid driver’s license with access to a reliable vehicle
