e4c is on the lookout for a Transitional Support Coordinator who resonates with our mission to eliminate poverty. Engage with participants facing various barriers and contribute to their recovery and stability. Your work will encompass life skills facilitation, wellness programming, and emotional support, all rooted in a trauma-informed, collaborative approach.
Key Responsibilities:
• Guide participants towards wellness and independence
• Facilitate crucial life skills development
• Support personal goal setting and achievement
• Create engaging wellness and life skills programs
• Respond to crises with effective risk assessments
Requirements:
• Educational background in Human Services or Social Work
• At least 2 years of direct service experience
• Certified in First Aid, CPR, and Suicide Intervention
• Experience in trauma-informed care practices
• Proven ability to work with interdisciplinary teams
