This critical position involves helping individuals create personalized, person-centered plans while you monitor their health and well-being. You will also ensure adherence to medical protocols, facilitate recreational opportunities, and mentor individuals in their daily lives. Key Responsibilities:

• Develop individualized support plans and document outcomes

• Provide direct care for high-needs individuals

• Monitor health, behavior, and emotional well-being

• Facilitate social and recreational opportunities

• Create objective records and documentation

Requirements:

• Diploma in a relevant discipline preferred

• Minimum 2 years’ experience in disability support

• Effective communication and mentoring skills

• Ability to model personal and social skills

• Understanding of available community resources
