Join a dedicated team at IHACSS to enhance the quality of life for those with complex needs. This role requires strong interpersonal skills and the ability to manage crisis situations effectively. Engage with clients daily, creating therapeutic relationships and supporting their journey towards greater independence and well-being.

Key Responsibilities:

• Support individuals in achieving their Individualized Care Plans

• Facilitate household maintenance and personal health activities

• Advocate for clients at medical appointments

• Provide crisis support and intervention when needed

• Monitor and report on individuals' progress and behaviors

Requirements:

• Degree or diploma in Health Services or related field

• Two years of relevant experience with adults

• Valid driver’s license and reliable vehicle

• Clean driving abstract and proof of insurance

• Vulnerable sector check and child intervention record check
