In this full-time role, you will provide individualized support, assisting clients in overcoming challenges and accessing vital resources. Key responsibilities include monitoring progress, establishing community ties, and fostering life skills. Collaborate effectively with the SIL Outreach team to ensure comprehensive care and administrative accuracy in documentation and reporting.
Key Responsibilities:
• Provide support hours as per PDD budget
• Monitor and report individual progress regularly
• Complete detailed case notes after every contact
• Establish community connections to support clients
• Assist with daily living tasks like medical appointments
Requirements:
• High school diploma required
• 1 year of experience with developmental disabilities
• Reliable vehicle and valid driver's license
• Satisfactory security checks including Vulnerable Sector Check
• Strong interpersonal and communication skills
Fulfill a vital support role assisting individuals in their journey toward independence and well-being.
