Outreach Support Worker

Make a difference as an Outreach Support Worker with GEF Seniors Housing in Edmonton, Alberta. Provide essential support to seniors navigating various challenges in their lives.

In this full-time role, you will report to the Community Support Manager, offering vital assistance to seniors living in GEF communities. Your responsibilities will include identifying support needs, fostering relationships, and collaborating with service providers to ensure comprehensive care. Building capacity and providing effective solutions are key aspects of this position.

Key Responsibilities:

• Identify seniors' support needs and create personalized plans

• Foster positive relationships between seniors and their communities

• Collaborate with agencies for comprehensive elder support

• Work with the Client Services Team on individual needs

• Maintain accurate documentation of service delivery

Requirements:

• Diploma or Degree in Human Services or related field

• 2–4 years of experience preferred

• Strong communication skills in English

• Valid driver’s license and vehicle required

• Advocacy skills with diverse populations
