Empower seniors as an Outreach Support Worker with GEF Seniors Housing in Edmonton, Alberta. Support vulnerable individuals while building meaningful connections and resources.
This full-time position involves working closely with seniors to understand their unique challenges and provide effective solutions. Reporting to the Community Support Manager, you will develop personalized support plans, collaborate with other services, and maintain accurate service records.
Your role is crucial in enhancing the quality of life for seniors.
Key Responsibilities:
• Identify and analyze seniors' individual service needs
• Create supportive community relationships for seniors
• Collaborate with social service providers for comprehensive support
• Work with the Client Services Team on individual assessments
• Ensure timely documentation of service delivery
Requirements:
• Diploma or Degree in Human Services, Social Work, or similar
• 2–4 years of relevant experience preferred
• Strong communication skills necessary
• Vehicle and valid driver’s license mandatory
• Compassionate advocacy skills for diverse seniors
