Key Responsibilities:

• Assess children and families to identify risks and needs

• Craft and manage case plans to support family wellness

• Provide crisis resolution and mediation services

• Connect families with vital community resources

• Track cases and report on progress periodically

Requirements:

• Social Work Diploma with 3–5 years of related experience

• Deep understanding of CKAM’s child welfare approach

• Proven experience in family-centered case planning

• Clean Class 5 Driver’s License

• Background checks required upon interview
