Transitional Support Coordinator Overnight Shift

Your role will focus on providing impactful recovery services to promote stability, wellness, and independence for our participants.
This full-time position is ideal for individuals with a degree in Human Services or similar fields. You will support people facing trauma and mental health challenges through life skills development and community engagement while using a strength-based and collaborative approach with interdisciplinary teams.
Key Responsibilities:
• Support participants with trauma-informed, person-centered care
• Facilitate life skills such as self-care and medication management
• Help identify personal goals and community connections
• Deliver impactful wellness programs promoting independence
• Maintain compliance with related standards and documentation
Requirements:
• Degree in a relevant field such as Social Work or Education
• 2+ years of experience in a supportive role
• Certifications in First Aid and CPR needed
• Familiarity with recovery and strengths-based practices
• Experience in collaborative settings with multiple stakeholders
