Reporting to Interdisciplinary Team Manager, the Adult Case Manager works within the Interdisciplinary Team Den and is responsible for the day-to-day care of community members in their den with emphasis on mature, adult issues. This includes trial entry into the den, through to goal planning, graduation, and post-graduation.

• Assist the Interdisciplinary Team Manager, in the daily operations of the den by working closely with community members in meeting their day-to-day needs.
• Support the creation of an inclusive,safeand friendly environment for all staff, communitymembersand external parties.
• Build rapport with community members byidentifyingstrengths and working in a respectful way with a demographic who has faced trauma and social exclusion.
• Maintainaccurateand comprehensive written documentation of all den community members related interactions in accordance withden standards.
• Maintain a strong knowledge of programs offered within the den and throughout the organization to be able to refer community members.
• Refer community members to community-based services and advocate on their behalf when needed inan appropriate manner.
• Deliver crisis intervention as needed, including risk assessment and follow-up until the situation has stabilized.
• Organize andfacilitatecase planning meetings, inclusive of entire den and community members.
• Maintain a professional and welcoming workspace andden
• Attend team and organizational meetings as required.
• Participate in required trainings and professional development.
• Complete any administrative duties as required.
• All other duties as outlined by program manager.
• All staff at BSCS are expected toparticipatein occasional outreach and emergency response efforts (e.g., winter response) to support and engage with clients.
• BSCS is more than a workplace. It is a community where everyone helps and contributes to the whole, and there is an expectation that all employees will take part in events (during the work week and occasionally during weekends and evenings). This includes fundraising campaigns, running community meals, medicine pickingetc.

• Class 5-non GDL driver’slicence, access to a reliable personal vehicle with $2,000,000 liability insurance, and a clean driver’s abstract
• Post-secondary Degree in a relevant field of study
• Minimum 6 months’ experience working with the vulnerable population
• Ability to respond appropriately in stressful situations and engage in de-escalation tactics when conflictarises
• Strong multi-tasking and organizational skills that can be applied to the use of Microsoft applications and dataanalytics
• Excellent knowledge and understanding of poverty related issues, trends, and services particularly, but not limited to the urban aboriginal population is essential
• Previous experience with crisis-interventionpreferred
• Police Check with the Vulnerable Sector and Child and Youth Intervention Check completed within the past sixmonths
• Valid First Aid certificate or enrolled ina futurecourse
• Experience with and knowledge of Indigenous culture and ceremonies is considered an asset.
