YMCA Hybrid Follow Up Support Worker

Lead change as a Follow Up Support Worker at YMCA, focusing on empowering clients through the Housing First model in Edmonton. Provide hybrid case management and community connection.

In this full-time role, you will enhance housing stability for individuals who have transitioned into homes via the Housing First program. With your trauma-informed approach, you will develop personalized action plans and conduct home visits ensuring clients succeed. Your work will make communities resilient and supportive.

Key Responsibilities:

• Deliver trauma-informed follow-up support and advice

• Create and manage Individual Action Plans for clients

• Conduct wellness checks to ensure housing stability

• Advocate for access to community resources

• Support clients in using public transit for appointments

Requirements:

• Degree or diploma in Social Work or related area

• At least 2 years' experience in social work contexts

• Valid driving license and access to a vehicle

• Exceptional organizational and communication skills

• Willingness to work flexible hours as needed
