Position: Support Coordinator for Individuals with Disabilities
Become a Support Coordinator with Winnifred Stewart and provide dedicated assistance to individuals with disabilities in their homes and communities. This role involves creating personalized support plans and ensuring individuals' well-being every day.

In this varied and impactful position, you will support individuals by documenting their person-centered plans and assisting in challenging situations. The role requires direct care for individuals with high needs, monitoring health conditions, encouraging rights awareness, and fostering community participation.

Key Responsibilities:

• Develop and document individualized, person-centered support plans

• Provide care for medically fragile individuals as required

• Monitor individual health and adapt support as needed

• Facilitate recreational opportunities within the community

• Communicate professionally with families and team members

Requirements:

• Minimum 2 years’ experience with diabetes or epilepsy

• Diploma or similar in a relevant field preferred

• Effective time management and coordination skills

• Well-developed communication and mentoring abilities

• Familiarity with Dual Diagnoses and Mental Health Supports preferred
