Join Winnifred Stewart as a Support Coordinator, providing direct assistance to individuals with disabilities while managing their daily home and community activities. This role emphasizes individualized care and support planning.
As a key member of our team, the Support Coordinator will design personalized plans for individuals with disabilities, promote community engagement, and ensure adherence to medical protocols. You will work closely with individuals, their families, and other staff to foster a supportive and inclusive environment.
Key Responsibilities:
• Develop person-centered plans for individuals and their support circles
• Assist with complex support situations and monitoring health
• Encourage individual rights and life choices effectively
• Provide direct care to individuals with high needs when needed
• Document and communicate all progress and conditions accurately
Requirements:
• Diploma or equivalent in a relevant field
• Minimum of 2 years experience with diabetes, epilepsy, or dementia
• Strong verbal and written communication skills
• Experience in team-oriented environments
• Positive attitude towards individual empowerment and inclusion
