• Rounding and managing the clinical care of residents at assigned SNFs.
• Rounding daily, prescribing medications and treatments for SNF residents.
• Providing high‑quality, patient‑centered, comprehensive primary care services.
• Timely documentation of all patient care/encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Documentation completed in Theoria’s proprietary EHR system, Chart Easy™.
• Use of Point Click Care , Matrix Care, and American Health Tech as appropriate.
• On‑site presence on scheduled shifts; reliable transportation required.

• Rounding time: 7 AM–1 PM local time, available to nursing staff.
• Shifts begin around 7 AM local time with reasonable flexibility.
• Daily shifts Monday–Friday.
• Charting/documentation can be completed off‑site.
• In‑person position.
