The Registered Nurse is responsible for overseeing coordination of the interdisciplinary plan of care and ensuring it aligns with the medical treatment plan.

The patient care process includes:

  • Assessment – a structured process of identifying, gathering and organizing subjective and objective data about the patient/family/significant other, including physical, psychosocial, spiritual and cultural aspects.
  • Planning – a step that identifies actual or potential problems and desired outcomes (goals), leading to a documented plan of care that directs the patient/family/significant other and the interdisciplinary team.
  • Intervention – the specific interventions or orders implemented in line with the plan of care and the medical treatment plan.
  • Evaluation – a step that evaluates the effectiveness of interventions in achieving the plan of care, determines conclusions regarding achievement or lack of achievement of desired outcomes, and may prompt reassessment and revisions.
  • Patients have problems identified and prioritized relative to biophysical, psychological, environmental, self‑care, educational and social status.
  • Goals are identified consistent with the problem to provide direction for patient care and health promotion.
  • Education is aligned with the identified problem and associated goals, viewed as critical to creating an environment that promotes patient empowerment.

Employee may be required to perform Limited Laboratory Services, including patient identification and preparation for specimen collection, sample processing, necessary quality control, instrument maintenance, monitoring, recording and reporting test results to clinicians, including critical values, and maintaining competency and certification for these tasks certainement.

Licensed and registered care providers responsible for utilizing the care process relative to their scope of practice and knowledge base may document assessment, plan, intervention, and evaluation. All patient‑care activities must be administered according to standards, procedural guidelines, and organizational policy.
