Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 shifts; flexible schedule, with 1-2 evenings until 7 PM.

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. Responsibilities include the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that allow effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves.

Responsibilities include:

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records in a clear, concise, accurate, and timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approvals.
• Participate in student program as assigned, providing a positive learning environment.
• Create an individualized professional development plan with clinical management.
• Adhere to discipline-specific State Practice Act and other regulatory agencies.
• Understand and follow the organization's performance improvement methodology.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR Certification.
