DescriptionMinimum Qualifications
- Master's degree in nursing from an accredited school or as grandfathered by NJ Law required.
- Current licensure as a RN in the State of NJ and certified by the State Board of Nursing.
- Certification within are of specialty required and granted privileges from ARMC medical staff.
- NRNNP, C or RNCNS, C registry unencumbered.
- ACLS required in Urgent Care, Atlanti Care Physician Group and Heart Failure Program
- BLS required.

Position SummaryWe are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant to join our Adult Medicine team. The NP or PA will work collaboratively with physicians, nurses, and support staff to provide comprehensive primary care services to adult patients, including those with complex chronic conditions. The ideal candidate will be committed to health equity, patient advocacy, and integrated, team-based care.
Key Responsibilities- Provide comprehensive primary care to adult patients (18+), including health promotion, disease prevention, diagnosis, treatment, and management of acute and chronic conditions.
- Perform physical exams, order and interpret diagnostic tests, and prescribe medications or other treatments as appropriate.
- Develop individualized treatment plans in collaboration with patients and the care team.
- Manage chronic illnesses such as diabetes, hypertension, COPD, and HIV, emphasizing education, prevention, and lifestyle modification.
- Collaborate with behavioral health, care coordination, and other team members to ensure holistic care.
- Document all patient care in the electronic health record (EHR) in a timely and accurate manner.
- Participate in quality improvement initiatives, case reviews, and team meetings.
- Adhere to clinical guidelines, regulatory standards, and FQHC policies and procedures.
- Maintain patient confidentiality and comply with HIPAA
- Strong clinical assessment and decision-making skills.
- Excellent communication, interpersonal, and organizational abilities.
- Commitment to working with underserved populations and understanding of social determinants of health.
- Ability to work effectively in a multidisciplinary, team-based care environment.
- Culturally competent and sensitive to the needs of diverse populations.
