Advanced Practice Nurse (PRN)

Ascend Health is seeking an Advanced Practice Nurse to join our South NJ hospice team on a PRN basis. The Nurse will conduct re‑certification visits, coordinate care, and support our interdisciplinary team.

• Conduct re‑certification visits to patients on hospice in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director.
• Facilitate access to appropriate support models of care (e.g., hospice, rehabilitation, on‑going aggressive treatment).
• Consult with the referring physician, acute care team, and other interdisciplinary members to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts, and documents.
• Assist in the development of clinical practice guidelines and standards to support quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors, and members of the IDT.

• Master's Degree in Nursing.

• Valid Nurse Practitioner license.
• 2–4 years of experience working within hospice or a related field.
• Valid driver’s license and reliable transportation.
