Lunet in Eindhoven zoekt een Persoonlijk begeleider die de bewoners centraal stelt. Je stelt individuele zorgplannen op en voert ze uit, observeert, overlegt en geeft advies, altijd in samenwerking met vertegenwoordigers.
Je coördineert de dagelijkse begeleiding en onderhoudt contact met verwanten, zodat de bewoners in een veilige en warme leefomgeving kunnen wonen. Je werkt tussen 07:00 en 22:00 uur in wisselende diensten; nacht- of slaapdiensten behoren niet tot het rooster.
