GGzE zoekt een Sociaal Psychiatrisch Verpleegkundige voor FACT Jeugd in Eindhoven. Je bent de spil in contact met ouders en betrokkenen, werkt outreachend bij crisissituaties en coördineert zorg rondom jongeren die vastlopen op school of thuis.
Je werkt multidisciplinair en draagt bij aan diagnostiek en behandeling. Het FACT-Jeugdteam werkt vanuit presentietheorie en herstelondersteuning, met focus op kracht en mogelijkheden van de jongere en omgeving.
