GGzE zoekt een casemanager voor de zorg rondom cliënten met bipolaire stoornis. Je fungeert als spil tussen cliënt, naasten en behandelaren en bewaakt continuïteit en afstemming in alle fasen van het ziektebeeld.
Je werkt in een multidisciplinair team, coördineert zorg, signaleert veranderingen tijdig en werkt outreachend bij crisissituaties. Je draagt bij aan zelfmanagement bij cliënten en betrekt naasten actief bij de zorg.
