WIJeindhoven zoekt een Generalist met SKJ‑registratie die inwoners in Eindhoven ondersteunt op alle leefgebieden. Je werkt in de wijk, met netwerk en partners en fungeert als vaste contactpersoon.
Je werkt integraal en zoekt samen met bewoners naar eigen oplossingen, regelt toegang tot Wmo‑begeleiding en kan bij complexe situaties mogelijk de regie overnemen als casusregisseur. Je werkt nauw samen met gemeente en zorgverleners.
