JOB SUMMARYThe Physical Therapist serves on a transdisciplinary team for families enrolled in the Infant Toddler Program. They provide comprehensive and integrated support as outlined in the Part C Program Manual for Infants and Toddlers in Kansas.
ESSENTIAL JOB FUNCTIONS Twain (RESPONSIBILITIES)- Collaborate with the transdisciplinary team to serve eligible infants and toddlers.
- Participate in team evaluations to determine eligibility of referred children and assess functional developmental strengths and challenges.
- Work with families and team members to develop individualized family service plans (IFSPs) based upon family-identified priorities.
- Provide family service coordination, including identifying, mobilizing, and coordinating community resources and services to maximize benefits.
- Collaborate with community resources for assigned families.
- Complete documentation for accountability, compliance, billing, data entry and communication with other providers according to state, federal, and logical guidelines.
- Deliver ongoing home‑ and community‑based intervention using a coaching model of caregiver support to integrate developmental activities into the child’s daily routines.
- Participate in teaming process, monitor parent‑child progress toward goals, mstain Rainbows mission, goals, and Part C Employee expectations.
- Perform other duties as assigned by the Program Coordinator or designee.
- Demonstrate ability to hear, understand, and distinguish speech and/or other sounds in person.
- Demonstrate ability to determine and identify items and children in the periphery, near, and far visual fields.
210 or 201 days per year (1,680 or 1,608 hours annually). Position is self‑paced; may require extended workdays and flexibility of hours.
Work EnvironmentRemote position that also operates in family homes and community settings as needed. Requires travel between RUI sites, family homes, and other community agencies.
Physical DemandsManage all duties with mobility, ability, and dexterity. Ability to lift up to 40 lbs. May work in noisy environments. Sitting 6–7 hours per 8‑hour shift, standing 1–2 hours, walking 0.5–1 hour. Lifting 1–10 lbs frequently; 11–20 lbs occasionally.
FLSA ClassificationExempt.
