Responsibilities
The Case Manager is responsible for having a thorough understanding of the patient’s treatment by communicating with the treatment team, advocating for the patient’s access to services during team meetings, and ensuring follow‑up care. The Case Manager also chairs treatment team meetings and conducts continued stay reviews as needed.
Qualifications- Education: Bachelor’s or Master’s degree from an accredited college or university in social work, mental health, nursing, or a related field (preferred).
- Experience: Minimum of two (2) years in a healthcare setting or managed care company with experience in patient assessment, treatment planning, utilization management, and/or case management (preferred).
