Position: Hospital Liaison for Long Term Care Facility

Acts as the primary contact between our healthcare organization and other hospitals, clinics, and health systems. Responsible for establishing and maintaining relationships, coordinating resident referrals, and ensuring seamless communication and services. Ideal candidate must possess excellent communication skills, a deep understanding of healthcare procedures and policies, and the ability to resolve issues quickly and effectively.

Essential Responsibilities/Accountabilities
  • Establishes and maintains relationships with hospitals, clinics, and health systems.
  • Coordinates resident referrals, admissions, transfers, and discharges.
  • Facilitates communication between the hospital, patients and their families.
  • Ensures the delivery of high-quality resident services.
  • Serves as a point of contact for external healthcare providers, such as clinics, doctors, and medical professionals
  • Assists with resolving resident issues and complaints and addresses concerns in a timely manner.
  • Provides information and support to residents and families about facility policies and procedures.
  • Collaborates with hospital and facility staff to ensure seamless resident care and service.
  • Organizes and conducts facility tours for residents and their families.
  • Facilitating meetings between residents, family members, and medical and nursing staff to discuss treatment plans and progress.
  • Assists in the development and implementation of resident care programs.
  • Keeps accurate records of resident interactions and interventions.
Job Requirements
  • 5 years proven experience as a Hospital Liaison Officer or similar role in healthcare.
  • Knowledge of healthcare procedures, regulations, and best practices.
  • Experience working with hospital social work and case management departments.
Job Skills
  • Excellent communication and interpersonal skills.
  • Ability to work under pressure and resolve issues.
  • Strong organizational and multi-tasking skills.
  • Proficiency in healthcare management systems and databases.
