Job Description

Reports to: Director of Crisis Services

Type: Full Time, 37.5 hours

Location: Community Based, Aurora

Schedule: Monday-Friday with 2 evenings or Saturday availability

Summary: Responsible for maintaining an active caseload providing services to clients and their families. SASS follow up services will be intensive, short-term, family and home/community-based services. SASS therapist/case managers are expected to comply with SASS Standards and Best Practices.

Essential Duties And Responsibilities

Clinical
  • Responsible for completing 15-1-hour appointments per week including but not limited to: therapy sessions, crisis shifts, and walk-in hours.
  • Responsible for completion of agency comprehensive assessments, individual treatment plan, progress notes for each client per FSA policy.
  • Responsible for utilizing Green space to monitor clinical benchmarks with clients.
  • Responsible for networking with collaterals on behalf of active clients, including: referral and linkage, discharge planning with in-patient psychiatric facilities, and the advocacy of a client’s rights within the education, legal, state, and mental health systems. This includes responsibility for coordinating services with our own sub-contracted agencies.
  • Responsible for responding to emergency/crisis situations with active clients and emergency back-up for SASS Assessments.
Supervision
  • Responsible for discussing cases with supervisor in clinical supervision (individual or group supervision) on a weekly basis.
  • Responsible for reporting to supervisor regularly about problem cases, need for special services and resource information.
Administrative
  • Responsible for completion of and meeting deadlines for all clinical paperwork for cases, including:
    Integrative Assessments and Treatment Plans (IM+CANS), Psychiatric Referral Forms, CIS Billing Notes, Case Closing, Illinois Medicaid Crisis Assessment Tool (IM-CAT), SASS transfer forms, and other clinical paperwork necessary or appropriate for maintaining case files.
  • Responsible for completion of and meeting deadlines for all agency administrative paperwork, including: CIS Billing Notes, time sheets, expense sheets, and any other administrative or agency requested reports.
  • Documentation/Billing Notes should be completed within 7 days of service.
  • Responsible for maintaining clinical charts in compliance with Rule 132 and Rule 140.
  • Responsible for attending all agency meetings, 9 team meetings per year, clinical supervision and any additional program meetings.
  • Responsible for participating in Utilization Review Meetings.
  • Responsible for serving on Agency and/or Program Committees when requested by supervisor.
Community Education/Public Relations
  • Responsible for educating the community regarding the SASS program in the service area.
  • Responsible for facilitating positive networking relationships with community service providers and/or networks as assigned by supervisor.
Training
  • Responsible for maintaining clinical charts in compliance with Rule 132 and Rule 140.
Other Duties/Special Assignments/Projects
  • Be available to help develop materials and participate in special community education projects/presentations as requested by administrative staff.
  • Pursue and maintain licensure and/or certification that are available and appropriate to educational/professional background.
  • Perform other duties as assigned by supervisor.
  • Perform incidental business-related travel as needed.
  • Participate in agency Risk Management and Continuous Quality Improvement activities.
  • Perform job responsibilities with a positive attitude.
  • Responsible for promoting collegiality at the workplace by working collaboratively with others.
